If your Department utilizes an ID number for you, please specify what type of number your Department uses :(required)

In this next section you need to advise us of your Qualifications/Certifications to list on your ID card. You can list a total of 5.

Do NOT feel obligated to fill out all 5 lines!! Just hit the MAJOR certs (IE: Firefighter I or II , EMT , Pump Operator , Fire Officer I or II , Fire Investigator or Inspector , Fire Police , ect. ect.)

Do NOT list minor certifications, especially if they were obtained under one of the major categories ( IE: ICS 100 , HazMat Awareness , Bloodborne Pathogens , ect. ect.) You need to list at least ONE major below. Any certs shown below will be shown on your ID in the same order. If we determine a certification is a minor we may use our discretion to show it on your ID or not.

In this next section we need your physical metrics for your ID. It would be best if you follow the same information as shown on your drivers license.

In this next section please let us know your department specific information.

IF YOU ARE SUBMITTING THIS AS PART OF A DEPARTMENT BULK ORDER ONLY ONE PERSON NEEDS TO FILL IN THIS SECTION !!!!!! However if you are submitting this as a single ID then please fill this out box completely.

This last section is for your specific medical requirements. This section is completely optional. If you do not feel you want to share this DONT, and we will not include a barcode on your ID. However on the fireground, when seconds count, having your emergency medical info at command officers fingers is very valuable! This information will be encoded into a bar code, and we will advise how to access it. We do NOT use QR Codes as they are not very secure.

At this point you are finished with the questionnaire to build your member ID Profile. BEFORE you hit the send button please look below at a few very important items.

We still need an ID Photo of you! You must email us a QUALITY digital picture of EACH member obtaining an ID. There are VERY strict guidelines that must be followed!

  • Picture MUST be from the CHEST UP. Do NOT send full body pictures!
  • NO HATS OR SUNGLASSES allowed. Prescription Glasses ONLY.
  • Make certain NOT to cut off the top of your head in the picture, leave some blank wall space above your head!
  • MOST IMPORTANTLY your picture background MUST be up against a BLANK SOLID WHITE WALL. Anything other than this will be rejected and delay your order! Also please make sure you have good lighting on your face.

Additional Items you may choose to supply if you choose. (If you do not provide any of these items we will not come looking for it and it will obviously not be included on your ID.

  • A QUALITY High Resolution digital image of your Department’s Logo
  • If your department already has an encoding standard for your medical information please provide that method if it differs from what we have specified we offer.
  • If your department requires an Officers signature on your ID card, you must provide that signature in a digital image format.

If you have ANY questions on these requirements, PLEASE CONTACT US! Be advised that certain information you provide WILL ABSOLUTELY BE VERIFIED! If you are a single individual requesting an ID we will absolutely contact the chief of your department to verify you are a member. We will also verify that the Department information you provided us is accurate and legitimate. We take security very seriously and so should you. PLEASE RESPECT THIS PROCESS! Take the necessary steps to advise your command officers that we may be reaching out to them to confirm the information you provided.
